ENROLMENT TERMS AND CONDITIONS
It is important that you agree to these terms and conditions below ticking the box on your child’s enrolment form. In consideration of enrolling my/our child at Grandma’s Place, I/We hereby agree that:
Orientation & Centre Policies and Procedures
1. I/We have visited the centre and discussed with the Director or Manager the enrolment of my child. I/We agree to notify the Director, Manager or delegated team member of any changes to the information provided on the enrolment form immediately when such occurs.
2. I/We agree to abide by the policies and procedures of the centre and acknowledge that we have access to a copy of policies and procedures, either in printed form in entry zone, or online portal, and will familiarise ourselves with these.
Finishing Enrolment
3. I/We agree to provide two weeks written notice of any intention to either withdraw my child from Grandma’s Place or reduce the number of days of attendance each week and that fees for these days are payable for this period. Failure to provide will result in my bond being forfeited, and I/We will be liable for any surplus unpaid fees.
Accidents & Emergency
4. I/We give permission for Educators and staff members of Grandma’s Place to provide and/or seek appropriate medical attention for the child in an emergency.
5. I/We understand that if at any time our child required emergency medical attention, Grandma’s Place will phone for an ambulance to transport our child to the closest hospital. I/We further agree that if at any time our child requires the administration of life saving medication (e.g. EpiPen or Ventolin), then the Director or other responsible staff member shall have the discretionary power to administer. We also understand that the Director will make every effort to contact us in these circumstances. I/We agree that if at any time our child requires emergency medical attention and neither parent/guardian can be contacted, that the Director or any other responsible staff member as agent for the parent shall have discretionary power to seek immediate medication attention at my/our cost.
Illness & Medical Issues
6. I/We agree that if our child is suffering from an infection, contagious disease or is in poor health, they are to stay at home until well, and that a medical clearance certificate may be required before being allowed to return to the centre. We understand that failure to do so may result in our child’s placement being withdrawn.
7. I/We agree that if our child is unwell, we will be required to collect our child immediately. If unable to collect our child immediately, we will ensure that we arrange for an alternative responsible person to collect our child. We understand that failure to collect our child in a timely manner may result in our child’s placement being withdrawn.
8. I/We agree to notify Grandma’s Place promptly of any absence and the reason for such absence and likely return date.
9. I/We agree that if our child requires any medication whilst in attendance, I/We will provide only medication that has a pharmacist’s label with the child’s name and dosage (even for over the counter medications) and that I/We will complete an Authorisation to Administer that Medication Form an arrival at the centre and that the medication will be handed to a responsible Educator or staff member. I/We understand that the only medication that will be administered without such written instructions is Panadol liquid (or equivalent paracetamol mixture) in the case of an emergency, or in the case of emergency situations, I/We agree that the centre may administer lifesaving medication (e.g. EpiPen or Ventolin) as outlined above in point 3.
10. I/We agree that Educators and staff are authorised to administer medication only in accordance with medical practitioner written instructions. In doing so, neither Grandma’s Place, it’s Management or Staff shall be liable for any allergic reaction or injury caused to the child by the administration of the medication in accordance with the Authority. Nor will they be responsible for any error contained in the Authority or the supply of incorrect medication by the parent.
Emergency Contacts & Custodial Arrangements
11. I/We agree to provide Grandma’s Place with the names and contact details of at least two responsible persons who can be contacted at the times when my child is in attendance, and are available to collect my child in the case of emergency or illness.
12. I/We agree to update Emergency Contact details in a timely manner when they change.
13. I/We agree to provide any court approved documents relating to our child in a timely manner and understand that in the absence of these documents, Grandma’s Place may be unable to act in accordance with your instructions.
Collection of Children
14. I/We undertake to have our child at Grandma’s Place no earlier than the stated opening time and no later than the stated closing time. We understand that failure to collect our child by the stated closing time will attract late fees, which are determined time to time. As at the date of this agreement late fees will be $2 per minute per family. These late fees will be paid by Grandma’s Place to the two staff members inconvenienced by lateness.
15. I/We agree that the child will be brought to and taken from the centre by a responsible adult and taken to the Director or other responsible staff member. I/We acknowledge that the person should generally be at least 16 years of age and should be able to provide proof of identity.
16. I/We agree that a child may be released in the care of a parent or responsible person who has permission, either as a nominated person in the Collection Arrangements, Emergency Contacts or persons nominated in other manners to collect the child on specific occasions.
Fees & Bonds.
17. I/We understand that statements will be emailed to me each week, and it is my responsibility to check weekly statements and advise of any concerns asap. If I/we wish to receive a printed copy of weekly statements I/we will advise Grandma's Place so this preference be accommodated.
18. I/We understand that a fee bond - equivalent to two weeks fees - will be required before the commencement date, and that fees must be paid weekly at all times, ensuring bond is kept at least two weeks in advance at all times.
19. I/We understand that fees remain payable, even when there is a public holiday or our child is absent through illness or for any reason. I/We understand that a discount is available for my child’s absent days when they are ill only when producing a Doctor’s Certificate for that period, and that the Doctor’s Certificate is required within one week of the absence. I/We understand that Grandma’s Place is unable to offer alternative make up days, unless there are vacancies in my child’s room.
20. Should our child not be able to attend for any reason, I/We agree to pay normal fees for the week. I understand that the Australian Government allows for up to 42 days absence per year (including public holidays) with Child Care Subsidy (CCS), and that if my child has more than 42 days absence per year my CCS is removed and I am liable for full fees.
21. I/We understand that if fees are not paid by the due date, my child’s enrolment at the centre cannot be guaranteed. I/We understand that late payment of fees may attract a $40 late fee charge and unsuccessful collection of outstanding monies may result in our names being registered with Credit Reference Association of Australia (CRAA) or other credit management organisations and legal action being initiated. I/We understand that should we leave with a debt and no agreed payment plan in place, Grandma’s Place will refer our account to a debt collection agency and that we will be liable for any associated recovery costs in addition to the balance owing.
22. I/We agree to pay an administration fee should our direct debit be dishonoured by our bank. This fee will be reviewed from time to time; at the time of this agreement this fee is $5.
23. I/We understand that Grandma’s Place has the authority to terminate enrolments at the Service in cases where fees remain outstanding for two or more weeks.
